The Medical Social Worker (MSW) is a qualified professional who provides medical social services and deals with the social, emotional, and economic status of patients in their homes.
Along with the Registered Nurses or Therapists, a Medical Social Worker will work with the home health care team and can:
- Counsel the patient and the family.
- Assess and evaluate the patient, family member(s), and the home situation to identify significant socio-economic and psychosocial status, including emotional factors that will affect the plan of treatment.
- Help patients utilize the resources of their families and refer to community resources such as:
- Meals on Wheels
- Non-emergency Ambulance Services
- Special Transportation Services
- Provider Services
- Medicare /Medicaid Enrollment Services
- Observe, record, and report the changes in the emotional and social factors that affect the patient’s illness, needs for care, responses to treatment, and adjustments to the care provided by the physicians and other members of the health care team.
- Assist the patient with advance directives.
- Assist in long-term care planning and /or placement to Assisted Living Facilities or Nursing Homes.
Our medical social workers hold a master’s degree from a school of social work approved by the Council of social work education, have two (2) years of experience with at least one (1) year of which was in a health care setting, a hospital, clinic, or rehabilitation center OR those who are in the process of acquiring certification from the Academy of Certified Social Workers.
