A patient may receive home health services if they are certified as homebound by a physician. A person is considered homebound when there is an underlying inability to leave home or when leaving requires a taxing effort. Our Registered Nurses (RNs) perform a comprehensive physical and psychosocial evaluation on their initial visit, also known as the Start of Care (SOC). During this visit, the RN reviews all medications currently being taken by the patient, assesses the home environment for safety, determines the need for a home health aide, physical therapy, occupational therapy, speech therapy, and medical social work services, and establishes a wound care treatment plan, if applicable. A plan of care, duly signed by the physician, is then established and periodically reviewed. All of our RNs are graduates of an accredited school of nursing and are licensed to practice professional nursing in the State of Texas.

Follow-up visits are performed by an RN or Licensed Vocational Nurse (LVN). Our LVNs are graduates of an accredited School of Vocational or Practical Nursing and are licensed to practice vocational nursing in the State of Texas.

Nursing care is delivered in accordance with the patient’s plan of care, which includes monitoring their overall physical condition; promoting health and preventing disease; coordinating services; conducting teaching and training activities; and providing direct nursing care.

Our nurses provide the following services in the comfort of the patient’s home:

  • Treatment and Injections (IM, SQ, ID)
  • Supervision of Medications
  • Dressing Changes
  • Wound Care/Wound Vac
  • Urinary Care (Foley Catheter, Condom Catheter, Straight Catheterization, Suprapubic Catheterization, etc.)
  • Ostomy Care (Colostomy, Jejunostomy, Ileostomy, Gastrostomy, Urostomy, Tracheostomy, etc.)
  • Lab Specimens/Venipuncture
  • IV Therapy: Peripheral, Central, Port-A-Cath
  • Diabetic Management and Education
  • Care of the Terminally Ill
  • Family/Patient Health Education
